The Carbon County Clerk’s Office is authorized to receive passport applications. We are a Passport Acceptance Facility designated by the US Department of State, Bureau of Consular Affairs.

General Requirements

  • Passport Hours – 8am to 4:30pm, Monday through Friday
  • Black ink must be used to complete the application
  • All applicants must appear in person at the time of application with required documents
  • Applications for children under the age of 16 require the presence, ID and signature of both parents. If the second parent is not available to sign, the appearing parent must present evidence that he/she has:
    • *Sole legal custody of the child, or
    • *Notarized written consent of the other parent for the issuance of the passport, see Form DS-3053

Required Items

  • Completed application (DS-11) which is available in the Clerk’s office or a fillable form is available online at travel.state.gov
  • Evidence of Citizenship which may include: Certified Copy of Birth Certificate, a Previous Passport, or Naturalization papers. This evidence is sent with the completed application
  • Proof of Identity – valid driver license, government, military, or state ID, or previous passport
  • One color passport photo – must be taken within the last six months. Our office does not offer this service. We suggest you have passport photos taken at one of the two local businesses that offers this service, Carbon Print and Design or Walmart. Call our office for more information

FEES

All fees are regulated by the US Department of State - Bureau of Consular Affairs.  These fees, payable to the US Department of State, are mailed with the application and must be in the form of a check or money order for each individual application. Fees for multiple members of one family can be combined in one check.

In addition to the fees listed below, each passport application has a $35.00 acceptance fee, payable to the Carbon County Clerk/Auditor. This payment can be made by cash, check or credit card. (Note: There is an additional 2.5% transaction fee for credit card purchases and we do not accept American Express.)

 

Passport Book - valid for all land, air and sea travel

Applicant 16 and older:

  • $130.00 - For Regular Processing
  • $130.00 + $60.00 = $190.00 - For Expedited Processing
  • $130.00 + $60.00 + current overnight mail charges - For Expedited and Overnight Processing

Applicants 15 and younger:

  • $100.00 - For Regular Processing
  • $100.00 + $60.00 = $160.00 - For expedited Processing
  • $100.00 + $60.00 + current overnight mail charges - For Expedited and Overnight Processing

 

Passport Card - valid for land border crossing or sea ports of entry from Canada, Mexico, Caribbean and Bermuda. For travel by water or land only!

Applicants 16 and older:

  • $30.00 - For Regular Processing
  • $90.00 - For Expedited Processing

Applicants 15 and younger:

  • $15.00 - For Regular Processing
  • $75.00 - For Expedited Processing
